Headsheet
The Headsheet portion of the database summarizes Medications, Problem List, Medical History, Surgical History, Social History, Family History, and Allergies. Addenda can be made at any time to any entry. Additionally, Pull down menus (combo boxes) allow minimal typing to complete the patient's record. The database program will also allow you to remove any item from patient's history or problem list, and it will be archived.
